Sustained weight loss maintenance is a constant challenge, especially for morbidly obese individuals who have tried and failed repeatedly. Why is it then, that some individuals are successful whilst others are not so successful? This two-phase sequential, mixed methods study sought to identify and gain insight into the factors associated with successful weight loss maintenance for participants in an adult weight management programme in the Waikato. Success appeared to be independent of age, gender, ethnicity, domicile, employment status and the presence of co-morbities.

Four major themes emerged:

- “Actions speak louder than words – just do it” (Following a lower energy diet, having higher levels of physical activity, regular self weighing, and vigilance in adjusting their behaviours based on the results)

- Routines were also important in supporting the changed behaviours

- What's it worth to me

- Valuing the achievements associated with lesser weight was seen as another important factor of success

This study examined the role of an advanced nurse practitioner in the identification, diagnosis and management of patients referred to an IBS nurse led clinic with symptoms of IBS as their primary problem. Items of interest were identification of the causes of symptoms, the effects of symptoms on quality of life (QOL), coping strategies of the patient, and patient satisfaction with care. These were measured by prospective data on two questionnaires given several months apart. In addition, some basic retrospective data were collected from a database of information on prior patients, to determine that the study subjects were representative of the IBS practice.

This thesis highlights the ways in which the practices of contemporary midwives in

Aotearoa/New Zealand are caught within the intersection of an array of competing discourses. The context for this is the reconstruction of midwifery in Aotearoa/New Zealand as an autonomous feminist profession founded on partnership with women. Interviews and participant observation with midwives, based mainly in one New Zealand city, are the basis of an analysis of the complexity of midwives? praxis as professionals. The analysis draws on insights from critical and feminist approaches to Foucault?s theories of discourse, power and the subject. It includes discussion of the conditions which came to produce and authorise the concept of ?partnership?. Which subjects can speak about partnership, and when? What claims

are made about it? What challenges it?

This study used an interpretive descriptive method to gain insight into and

explore key issues for rural nurses working and living in the same

community. Four Rural Nurse Specialists were recruited as participants. The

nurses had lived and nursed in the same rural community for a minimum of

12 months. Participants were interviewed face to face and their transcribed

interviews underwent thematic analysis.

The meta-theme was: the distinctive nature of rural nursing. The themes

identified were: interwoven professional and personal roles; complex role of

rural nurses and relationships with the community. A conceptual model was

developed to capture the relationship between the meta-theme and the

themes. A definition for rural nursing was developed from the findings.

This research identified some points of difference in this group of rural

nurses from the available rural nursing literature. It also provides a better

understanding of the supports Rural Nurse Specialists need to be successful

in their roles, particularly around the recruitment and retention of the rural

nursing workforce.

BACKGROUND: Emergency nursing is a specialty concerned with the care of people of all ages, with either perceived or actual unwellness presenting to the emergency department(ED) for assessment, resuscitation, investigation, treatment and review of their illness or injury. Emergency nurses apply specialty knowledge and expertise in the provision, delivery and evaluation of emergency nursing care. Over recent decades social, political and professional changes have affected nursing care delivery and nursing education. In particular the 21st century has witnessed the development of state funded postgraduate nursing education programmes, developing nurses specialty or advanced nursing knowledge, quality patient/client care and nursing practice within the tertiary education system.

AIM: The aim of this study is to investigate emergency nurses? perceptions of the impact of postgraduate education on their practice in New Zealand (NZ).

METHODS: This study utilises critical social theory as the overarching framework, informed by the writing of Jürgen Habermas (b.1929- ). It is the three phases of

Habermas?s practical intent of critical social theory; namely enlightenment, empowerment and emancipation, that this study is concerned with. This descriptive research study employs both quantitative and qualitative methods and is therefore known as mixed-methods research. Data collection took place over 12 weeks, from August to November 2006, using a survey questionnaire obtained with permission from Ms Dianne Pelletier, Sydney, Australia. The sample included 105 emergency nurses from District Health Board (DHB) emergency departments in NZ, 10 respondents from this sample self-selected to be interviewed by telephone. Ethical approval for this study was obtained from the University of Otago Ethics Committee for research involving human participants. Data was analysed using the Statistical Package for Social Sciences (SPSS).

RESULTS: Two main themes arose from the thematic analysis; these being positive and negative, these themes were further divided into 10 sub-themes. The results indicate that postgraduate study (PGS) has increased nurses? perception of their knowledge; leadership and understanding on the quality of patient care delivered, increased their academic and research skills and increased their confidence/self-esteem and recognition by their colleagues and team. Therefore the majority of respondents perceive postgraduate education has been an instrument of liberation and a process of empowerment and emancipation. A smaller percentage of respondents perceived that PGS had no effect on various aspects of patient care and another significantly smaller percentage of respondents reported negative results from PGS. This research identified similarities between this study and that of Pelletier and colleagues? (2003; , 2005; , 1998a; , 1998b) Australian study.

CONCLUSION: This study adds to the existing literature on postgraduate studies undertaken by nurses. No known study has previously investigated solely emergency nurses?perceptions of the effects of PGS, either nationally or internationally. The results of this study offer enlightening information regarding emergency nurses? perceptions of their PGS within NZ and offers a platform from which other studies may be undertaken. It also has the potential to inform nurses contemplating PGS and educators facilitating these programmes,as well as provide implications for policy development by the Nursing Council of NZ, NZ Universities, DHBs and the Ministry of Health.

  Title Revisiting the past: A focused ethnography of contemporary dual diagnosis nursing practice Type

  Abstract (up) As has been the case internationally, deinstitutionalisation of dual diagnosis (intellectual disability and mental illness) services has also occurred in New Zealand. Inpatient services have been redefined to respond to the more acute focus that has arisen out of this deinstitutionalisation process and nurses are having to redefine their roles in response. This study was undertaken to explore and describe the culture of nursing practice in a dual diagnosis inpatient unit in one psychiatric hospital. A focused ethnographic approach was used to triangulate data gathered from fieldwork observations, review of documents and semi-structured interviews. Schein's (1985) levels of culture model, was used to identify and explore the artifacts, values and assumptions evident in this nursing practice. Analysis presents three key themes categorised as 'communication', 'assessment' and 'safety'. While these key themes are shown to be evident in the everyday practice of the nurses, how these relate to the notion of 'dual diagnosis nursing' is not clear. Therefore, the author describes the major finding of this study as revealing a nursing culture holding tight to traditional psychiatric and psychopaedic nursing practices and struggling to develop a distinctive culture in the absence of a defined dual diagnosis knowledge base. The author concludes that these findings suggest an urgent need to provide nurses with support in gaining contemporary knowledge regarding dual diagnosis nursing. Support for nurses in advancing these areas then impacts on support for the patients. It is suggested that additional research is undertaken to assess the learning needs of the nurses in order to develop clinical practice guidelines for this area. Further recommendations are made to address system issues which are contributing to the gap in knowledge.

  Abstract (up) Baker et al., (2008) showed diminished rates of survival in the CPR-first group. Based largely on the evidence of the two Australian RCTs, the 2010 ILCOR guidelines removed the recommendation for CPR first, stating that 'there is inconsistent evidence to support or refute a delay in defibrillation to provide a period (90 s to 3 min) of CPR for patients in VF/pulseless ventricular tachycardia (VT) cardiac arrest' (ILCOR, 2010, p. e6).\n For this reason, and due to the increased chance of accidental defibrillation, it is the recommendation of the author that it only be used by health professionals who are able practise on a regular basis.

  Abstract (up) Eighteen months after the first of many large earthquakes, Christchurch-based home health care provider Nurse Maude surveyed staff to identify the impact on the well-being of their mainly elderly clients. Responses from 168 staff identified five key issues. These were: mental health, anxiety, and depression, symptoms similar to post-traumatic stress disorder (PTSD); unsafe environments; loneliness and isolation; difficulty coping with change; and poor access to services. To meet the needs of clients in this challenging environment, staff felt they needed more time to care, including listening to stories, calming clients and dealing with clients who had become slower and more cautious. Damaged and blocked roads, and the fact that many clients moved house without warning, added to the time it took to deliver care in the home. This survey has helped Nurse Maude build on its initial post-earthquake responses to better meet the needs of clients and support health-care workers in this stressful environment.

  Abstract (up) Examines the lived experiences of adults with myasthenia gravis(MG). Uses an interpretive phenomenological approach applying the research methodology of van Manen (1990). Interviews seven people living with MG and records their experiences of the disease. Poses broadly-worded questions about various topics related to MG, that include diagnosis, symptoms, treatments and coping strategies, guided by individual experiences. Reveals by means of thematic analysis that MG affects every aspect of a person?s ?lifeworld?: their sense of time, body, and space and their relationships with others. Highlights three main themes embedded in the data experienced by a person with MG: living with uncertainty, living with weakness and living with change.

The research project sought to describe the flight nurses perceptions about the factors that influence clinical decision making in their flight nursing practice, using a descriptive survey methodology. Themes emerged as factors which participants perceived influenced clinical decision making in their flight nursing role and in the aeromedical role. These themes included pre-flight preparation, patient status, experience and education of the nurse, and the challenges associated with the physical and atmospheric environments.

  Abstract (up) Identifies the experiences of NZ cancer nurses whose primary role is to care for patients aged 20 or older, and their whanau/family, and describes the factors that may influence care. Examines whether nurses received training in the management of stressors associated with caring for cancer patients, either during their training or while in the cancer workplace setting. Considers whether nurses working in peripheral (satellite) cancer centres were at more risk than their colleagues in larger regional centres. Conducts a quantitative, descriptive and anonymous survey of members of the Cancer Nurses' Section of the NZNO, using the Professional Quality of Life (ProQOL) questionnaire, which scores compassion fatigue, compassion satisfaction and burnout.
