The mnotivation for professional nurses to chose entry to these particular professions may in some way throw light upon the difficulties being experienced in attempts to bring about changes of an emancipatory nature in the nursing profession (Habermas, 1974). The approach for the study is hermeneutic phenomenology (Gadamer, 1975 – ; Heidegger, 1962)

  Abstract The original purpose of this work was to put some notes together about nurse training at Sunnyside for the hospitals 150th anniversary celebrations. When the celebrations were cancelled, the author continued with the project out of personal interest.A variety of historical research methods were used to collect data for the project. These included a literature search of historical publications related to the subject and the period; gathering documents and statistical records from a variety of sources; writing letters to previous students and tutors, requesting photographs anecdotes and opinions; and a number of prearranged interviews.The resultant brief history, published finally in 1992 is a collection of records, anecdotes, reproduced documents and photographs, interspersed with referenced information from the literature and the writers comments.Past and present staff of this once large psychiatric institution have been able to look at their training in a historical and social context, and in many cases, to see their names in print in the class lists. Others who are undertaking a historical study of Mental Health Nursing in New Zealand may find it useful as a reference work

How nurses understand family affects their practice with families facing the diagnosis of childhood cancerShaped by Heideggerian phenomenology, van Manens methodology for hermeneutic phenomenology was used to construct an understanding of family from the experiences of family members facing the diagnosis of childhood cancer. Seven family members from two families, one mother, two fathers, two siblings, and two grandparents were interviewed about their experience of facing the diagnosis of childhood cancer.From the participants experience the meaning of family was interpreted as being-with-others, for-the-sake-of-others, who one might not distinguish from oneself. This understanding of family is recognisable, yet different from traditional definitions of family and may help nurses and family members to act more thoughtfully and tactfully with each other

  Title A study of cardiac inpatient participation in Canterbury Health's cardiac rehabilitation programme Type

  Abstract A telephone survey was conducted with 61 randomly selected patients who had been admitted to the cardiology service with an Myocardial infarction but did not attend the cardiac rehabilitation programme. Findings show that many patients who would benefit from cardiac rehabilitation are denied access because it is only open to patients who have Myocardial Infarctions. Only a small percentage of MI admissions attend rehabilitation the main reason being patients are not informed the service is available. Women are under represented among rehabilitation participants and lack of transport is a major factor inhibiting women from attending. Improvements in referral systems and follow up are recommended

  Abstract This action research project explored the factors that helped or hindered student nurse clinical learning from the perspective of nurse educators, practitioners and students. Participant analysis of their own discussions identified both common and disparate views regarding the student's learning experience. Researcher analysis identified five practical and three organizational issues that influenced the development of more effective partnerships between these three stakeholder groups that would facilitate student clinical learning. The practical issues were how to deal with the 'problem' people in the learning process, how to clarify and develop the various roles in the learning context, how to generate more effective communication, how to respond more effectively to the impact of the changing environment, and how to maximize 'moments of learning'. The organizational issues were identified as the schisms between the disparate personal and organizational cultures that direct the way educators, practitioners and students, perceive, think, feel and act

  Title Realities and rhetoric: general hospital nursing in New Zealand 1945 to 1960 Type

  Abstract Up until the 1980s most of the historical writing about nursing and nurses in this country has been told from the points of view of past nursing leaders. The realities of day-to-day nursing in New Zealand general hospitals were relatively unknown.This thesis examines the experience of general hospital nursing between 1945 and 1960. The recollections of thirty-four nurses who nursed during this period have provided the key sources from which the major themes of this study have emerged. These themes, of dirty work, authoritarian control and discipline, and learning nursing are discussed within the context of an expanding hospital system and a shortage of nurses.The study demonstrates the vast differences between the recollections of nurses of the experience of nursing and the rather high-flown rhetoric of the nursing leadership.Changes to the amount of cleaning, the ;level of discipline and control, and ways in which learning nursing was organised were small and gradual and occurred in the late 1950s.Overall, though, nursing in general hospitals by 1960 ws almost unchanged from the 1930s.An ethos of selfless service, opposition to unionism, and Christian altruism was till dominant amongst the nursing leadership. Nurses in training still worked a six day week, were expected to stay on duty until the work was done, and were supervised closely in, on and off duty time

  Title Caring for the dying adolescent: The lived experience of four mothers Type

  Abstract The aim of this thesis is to present a phenomenological exploration of the experiences of four woman who have had an adolescent child die of cancer. Using the method of hermeneutic phenomenology, as described by van Mann (1984,1990) the study drew on the stories of four woman. From their stories eight emerging themes became apparent.The analysis of the themes is described under the following headings:- wanting to protect; wanting to be with; maintaining normality; feeling positive and hopeful; facing finality; realizing that death was imminent; coping with the common reality; and interacting with health professionals.Further interpretation of the emerging themes revealed “hope expectations” as the phenomenon of concern that captures these mothers' experiences. The hope expectation phenomenon reveals how the mothers coped with adversity, differing hope expectations in the common reality, oscillating hope expectations throughout the journey and hope expectations inspired by the health professionals.The outcome of this work is to provide professional, especially nurses, with insight and knowledge of the maternal experience of caring for the dying adolescent. It is hoped this research will contribute to the body of nursing knowledge and influence the care of parents who experience the trauma of caring for their dying adolescent child

  Abstract This phenomenological study described the lived experience of patients within a surgical secondary care setting, and explored patients' perceptions of nurses' practice. The participants in the study were five adult patients who experienced a period of hospitalization within the surgical secondary care setting.This study was influenced by the work of Patricia Benner (1984) who entered the lived world of nurses' practice, through description of nurses' clinical exemplars. In this study, a qualitative research approach was used to enter the lived world of the patient within the surgical secondary setting.The research approach followed Max Van Manen's phenomenological hermeneutic method of 'Researching Lived Experience". This approach includes turning to a phenomenon of concern and interest to the researcher, investigating the experience as it is lived, reflecting on essential themes that emerge from the investigation, describing the phenomenon and bringing it to speech. The data that emerged from the participants stories of their experiences with the surgical secondary care setting, revealed four essential themes. These themes included 'feeling valued', 'human to human contact', 'two way communication', and 'feeling safe'. These themes provided insights into the participants' perceptions of nurses' practice.The participants' stories revealed a range of perceptions about nurses' practice, and the extent to which nurses in their practice valued the patient, had human to human contact, kept the patient informed and ensured patient safety. The themes revealed times when nurses' practice was perceived as dependent and powerless, in the face of, traditional views of womens' work, and the hierarchical and beurocratic constraints of the institution. At times it was also hidden from view, missed or misunderstood. Despite this, nurses' practice was seen as the central and pivotal point, from which the participants were able to make their recovery

  Title Critical case studies as voice: the difference in practice between enrolled and registered nurses Type

  Abstract An emergent theoretical perspective in the nursing literature relates to nursing as a human science and the application of critical social science and feminist frameworks to nursing research. This research adds to the discipline's knowledge base in that it is at the leading edge of exploring how critical case study illuminates critical issues in the practice of nursing.My interest in the research question came from hearing colleagues espouse the view that other than a legal one, there were no differences in practice between enrolled and registered nurses. How accurate was this perception? Without answers to this question, members of the nursing profession in New Zealand were likely to make some major decisions about the future of enrolled nursing which history will show were ill founded. Since I “walk the talk” of both the world of practice and the corridors of nursing power relationships, I sought answers by incorporating the viewpoints of both groups into the research design.Five registered nurses who had previously been enrolled nurses were asked to consider what was different about their present practice compared to their experiences as an enrolled nurse.Through critically reflexive discourse and journalling, Liz, Emma, Kathy, Helen and myself came to reflect on our nursing worlds in order to transform them.Writing about these transformations in a way that kept our voices alive was more difficult. The cases were written as stories, using storytelling as a legitimate academic activity to link the methodology with the theoretical perspectives. This ensured our individual voices were heard rather than silenced by the research process. It also left a clear decision trail for the reader to follow related to issues of rigor. Paradoxically, the identification of patterns across the case studies was facilitated.Two main patterns emerged in answer to the question “what's different in your practice now from when you were an enrolled nurse?” The differences were explicated in the pattern “Becoming a registered nurse” while “She was one of us” exposed the underlying power and control issues. This study found that the enrolled nurse controls practice at the bedside.The nursing profession's stance on the future of enrolled nurses was also explored through case study. Key nursing stakeholders were interviewed using a proposed legislative change to the Nurses' Act, 1977, that would lead to the demise of the enrolled nurses as a category of nurse, as a focus point. These different perspectives expose for critique the socio-political forces that silence the voice of enrolled nurses in determining their own future. At a legislative level, the stakeholders' voices are dominant. The future of enrolled nurses looks increasingly bleak in New Zealand as the nursing profession positions itself legislatively for the 21st century

  Title Constant vigilance: the lived experience of mothering a hospitalised child with acute illness or injury Type

  Abstract This phenomenological study describes the lived experience of mothering a child hospitalised with acute illness or injury. Seven mothers who had experienced this crisis within twelve months of our first interview agreed to share their stories with me. The resulting data were analysed and interpreted using van Manen's interpretation of Heideggerian phenomenology.Four phenomenological themes emerged from this study. Mothers have a special kind of knowing. They have a need to do with and for their child. Handing over to or leaving their child in the care of strangers and waiting for their child to be returned to their care are very difficult things for mothers to do. Their constant vigilance is enabled by their special kind of knowing and their need to do. The difficulty of handing over, leaving and waiting is emphasised by mothers' constant vigilance.Personal experiences during the course of my study presented significant challenges to my ability to offer an effective phenomenological description of the phenomenon under study. Continuous reflection aided by dialogue with fellow phenomenological researchers has resulted in a meaningful narrative.This description of mothering in a context of crisis is useful in the potential contribution it makes to nurses' understanding of mothers' experience of the hospitalisation of their children. It supports the philosophy of family-centered care and highlights the ability of individual nurses to make a positive difference to a very stressful experience
