  Abstract (up) Discourse about women assuming greater control over their childbirth experience has received much attention in contemporary childbirth and womens' studies publications. Research in the domain of childbirth has shown that a sense of control over one's birth experience is related to childbirth satisfaction. More specifically, studies have used Locus of Control measures and have found a significant relationship exists between expectations for control and choice of birth place. Choice of birth place features in some articles as a factor that determines the degree to which control can be exercised. Home, as a place of birth, is considered a setting in which a woman can have optimum control over her experience. Many women are discouraged from taking up this option, based on fears about safety. Reflection on why women continue to pursue a home birth against the wishes of their care giver rekindled and interest in Locus of Control. This study was undertaken to explore the relationship between urban women's choice of birth place setting and their scores on two different measures of Locus of Control (LOC). The aims of the study were: to test whether LOC scores predict choice of birth place for women who chose hospital, home or a birthing centre as their place of birth; to test which of the two LOC scales (Levenson's generalised measure or Wallston et al's more specific scale) is the most useful in predicting the influence of LOC and choice of birth place and to explore the relationship between choice of birth place LOC, demographic and contextual factors surrounding pregnancy. The method used in the study was a descriptive correlational design. One hundred and ninety four pregnant urban women who accessed a range of maternity care providers in the greater Auckland , Wellington , Christchurch and Dunedin cities responded to a “Birth Choice Survey” between April and June 1996. Tests of significance found significant differences for powerful others and chance Locus of Control among the three groups of women who chose hospital, home or a birthing center as a place of birth. Differences were significant at p = 0.05. No significant differences were ffound in the scores for internal Locus of Control for women in the three groups. Limitations in the sample sixe prevented linear discrimination being successful in predicting the influence and choice of LOC variables and choice of birth place for both scales. A classification tree was employed to identify key variables which might affect choice of birth place and indicated that women who were extremely happy with their birth place choice were more likely to be those who chose home birth. Locus of Control measures are interesting, and if used specifically, may be helpful in relation to understanding a women's choice of birth place. However general assessment of the contextual factors appears just as relevant. Implications of this study are the while women may differ in how they perceive their outcomes are determined, an overwhelming majority of women who participated in this study value their ability to make choices about their birth location. Identifying consumer perceptions about control and the contextual factors that influence women's choice of their birth place has the potential to clarify women's preferences for responsibility in their birth experience. Limitations of the study were addressed and included suggestions to improve response rates and thus generalizability of findings. Implications for further research were also considered

  Abstract (up) Maternity services in New Zealand have undergone some fundamental and unique changes over the last few years. The promotion of competition among providers for clients have had repercussions on the way that maternity hospitals offers services to women. In adapting to this change, it is important that 'the baby is not thrown out with the bathwater'. Unfortunately the most commonly used indicators, and the standard “Patient Satisfaction Survey”, neither of which contain many valuable clues for maternity facility providers to determine the quality of their service from the consumer's perspective. The need was identified for the development of quality indicators to evaluate maternity hospital care. Closely related to this was the need to determine of how dependable providers are at determining what consumers would describe as an optimal maternity hospital service

  Abstract (up) The aim of this qualitative descriptive study was to explore the experience of new comprehensive nursing graduates as they adapted to working in the acute psychiatric setting. Interviews were conducted with four participants, focussing on their current work experiences and how the philosophical beliefs and values derived from their educational preparation fit with those they encountered within the practice setting. The data were analysed by noting common experiences, values and meanings and identifying the themes as they emerged. The themes were: transition to practice, conflict, contradiction, structural constraints, and the reality of the psychiatric setting. The results of the study confirm the concern that has been voiced by new graduates about the quality and quantity of current orientation programmes. Conflicting values and beliefs concerning the nature of mental health/psychiatric nursing has also become evident. It appears that the graduates' Comprehensive nursing preparation may have contributed to their feelings of unease as they attempted to fit their own values and beliefs about nursing with those of the acute psychiatric setting

  Abstract (up) The purpose of this study was to determine if there was commonality amongst teachers, in perceptions of the role of student self-assessment in the nursing programme. Four nurse educators, who were teaching and supervising students undertaking self-assessment in clinical practice, participated in a semi-structured interview. Three focus questions guided the interviews and addressed the individual teachers perception of self-assessment, what the teacher considered the benefits of self-assessment to be, and how the teacher used student's self-assessment's in their role as clinical supervisor. The taped interviews were transcribed, and coded and general themes identified. The findings revealed a high degree of consensus amongst the teachers in regard to the co-operative nature of self-assessment, and the teacher's role as a facilitator. The need for students to develop skills of analysis and reflection on their performance as well as the need to furnish students with clear guidelines on what to self-assess, were highlighted

  Abstract (up) The study reconceptualises the process of nursing practice where health is expanding consciousness. The praxis methodology and design derive from the findings of the previous study (Litchfield, 1993) through which a framework for personal practice was articulated. The philosophical premises were hermeneutic and dialogic reflecting a narrative orientation within a participatory paradigm. Ontology and epistemology merge and language is fundamental. The findings from this subsequent study depict the process of modeling practice as a tetrahedron to show inter-relatedness of four facets, each defined completely by the others: partnership, dialogue, pattern recognition and health as dialectic. Five young families with complex health circumstances were preferred by Plunket Nurses and visited at hole to talk about health and the family. Th e process of health patterning ended with indication of insight as the potential for action; the partnership ended as the closure of the initial contract to provide a summary text to the family. Transformative change in family living was identified. The continuous analysis of the scripts of the evolving conversations and summary text showed the relational, dialogic processes were identified as vision – finding purpose to act in the here-and-now against the backdrop of past and potential of the future; and community – a sense of being connected, participant and relevant in society. This process of research, as if practice, presented health and caring as synonymous and core of the discipline of nursing

  Abstract (up) This action research project explored the factors that helped or hindered student nurse clinical learning from the perspective of nurse educators, practitioners and students. Participant analysis of their own discussions identified both common and disparate views regarding the student's learning experience. Researcher analysis identified five practical and three organizational issues that influenced the development of more effective partnerships between these three stakeholder groups that would facilitate student clinical learning. The practical issues were how to deal with the 'problem' people in the learning process, how to clarify and develop the various roles in the learning context, how to generate more effective communication, how to respond more effectively to the impact of the changing environment, and how to maximize 'moments of learning'. The organizational issues were identified as the schisms between the disparate personal and organizational cultures that direct the way educators, practitioners and students, perceive, think, feel and act

  Abstract (up) This action research study explores the experiences of 'patient education' from the perspective of a group comprising two nurses, two people with asthma, and the researcher who is a nurse who has asthma. The method used is emancipatory action research (Grundy, 1990) with critical social theory and feminism as theoretical underpinnings.The focus moves from patient education to a broader view of living and working with asthma. The story of the group is presented in the format of a play. A play within the play tells of living and working with asthma.An action research spiral is formed which reflects the way the group moves through the three modes of action research described by Grundy (1990). The acts of the play represent each of the stages of the action research process. The emphasis moves from power and control through to practice wisdom.The main issues explored are: the nature of patient education by nurses; the implications this has for relationships with patients and nursing education; power and control in the secondary setting; the lived experience of chronic illness and the practice wisdom of nurses and people with asthma. The work concludes with recommendations for change in each of these areas based on the work of the group

  Abstract (up) This paper outlines a phenomenological study carried out to explore the experience of nausea, vomiting and retching associated with pregnancy. These phenomena lack Nursing research and are debilitating symptoms which effect the quality of life for 50-75% of all pregnant women. Madjar ( 1991) indicates that the communication of such experiences can deepen our understanding of human life and coping. It is importance for Nurses to develop an understanding of lived experiences so that they can make more effective interventions. This study explores the essential humanness of life experiences as they are for those who live them. It involved collecting data from those experiencing the phenomena and analysing it. It focuses on the study of phenomena not as separate entities in themselves but as they are perceived as they are experienced. A 'purposeful sample' was required for this study. The aim was to include women who had direct knowledge of the phenomena of nausea, vomiting and retching associated with pregnancy. The study involved in depth interviewing of six women, the interviews were taped and independently transcribed, the transcripts were analysed to determine the meaning of the experience and to identify common themes. The experiences of the women are discussed in relation to what van Manen (1990) describes as four main life world existentials; the lived body, the lived other, lived space and lived time

  Abstract (up) This phenomenological research project focused on one womans experience following total removal of her larynx and aimed to faithfully capture the essential structure of the phenomenon of voice as it emerged for her. For women in particular gaining a new artifical voice following surgery that is considerably lower sounding, plus the large visible opening in the lower neck is psychologically challenging and can be socially isolating. Most literature relating to laryngectomy and rehabilitation comprises quantitative research with limited attention to womens issues. This could be attributed to the small number of women undergoing this surgery. A review of nursing literature revealed minimal research, with none referring specifically to women. Nor were any studies using entirely qualitative methods identified.This research project used a phenomenological approach informed by the work of Michael Crotty, described as within the parameters of mainstream phenomenology. Based on a traditional common core foundation which is critical, holistic and individualistic, mainstream phenomenology requires achieving co-researchership between researcher and participant while exploring phenomena as objects of authentic human experience.The study began with discussing Crottys stepwise method emphasising the activity of bracketing. The co-researcher was then left to ponder or meditate on her experience following bracketing all her prior understandings and ideas, as if this was the first time she had encountered the phenomenon. Three conversations followed which allowed exploration of the co-researchers written statements. The seven interwoven dimensions that emerged represented the essential structure, meaning and essence of 'what voice is for this woman following total laryngectomy

  Abstract (up) This research aims to illuminate the reality of health and health maintenance as four postmenopausal women understand and experience it. The research employs feminist methodology, informed by postmodern feminist writers, in order to allow maximum flexibility and to be congruent with supporting the exploratory nature of the research. The women in the study self-nominated as postmenopausal, and functioned equally as co-researchers with the researcher in a focus group which met twice for one-hour audiotaped interviews. The resultant combined statement on health was produced after thematic analysis of the data and individual reflection, and represents the values these women hold, that health is largely attitudinal.“Health is not what happens to you physically, emotionally or mentally, but how you deal with it.”It is notable that the women did not mention nurses as having a role in their health maintenance, nor were other health professionals considered to be essential. As well, hormone replacement therapy – a common theme in women's conversation and lay women's literature – was not mentioned as being an integral to the women's health maintenance. The methodological approach means that the research in non-generalisable. However the findings may add to existing knowledge about prevailing health concerns of postmenopausal women, may enhance the discourse, and may identify the need for further research

  Title Nurse meeting another: cultural safety in nursing practice Type

  Abstract (up) This research project, a descriptive study using narratives, explored the application of cultural safety theory and philosophy to clinical nursing practice. This application was illustrated through the stories of four experienced Pakeha,Tauiwi registered nurses in Aotearoa/New Zealand, who described their realities of applying cultural safety to daily clinical practice. The incentive for this study had been identified in light of the current political climate, pragmatic realities and in keeping with the current state of knowledge.Cultural safety was first identified by Maori nursing students and subsequently described and articulated by Maori nurses, as being a potential solution to improving Maori health statistics in Aotearoa/New Zealand. Many Maori and Pakeha/Tauiwi in this country accept that the Treaty of Waitangi, a covenant signed between Maori and the Crown in 1840, is the incentive for giving cultural safety status and credibility. While the Nursing Council of New Zealand has supported this concept and made it a requirement for all nursing education, there is little literature written concerning its application to practice from a Pakeha/Tauiwi perspective.The perceptions and insights of these nurses were heard when they were invited to describe how they saw cultural safety as part of their daily clinical practice and recount their struggles, realities, practice and experience. The literature review supported the use of narratives as an appropriate method for this study. The philosophy and assumptions of narrative appear to match the oral tradition of nursing and thus it was considered possible to contemplate the fit of narrative to nursing research.The stories of these nurses, gave examples of best nursing practice in which cultural safety was integral to practice, and provided exemplars of possible beginnings and possible endings. The depiction of cultural safety in practice surfaced as the weaving of four themes which were consistent in all the stories – themes of reflection, reverencing, the environment, and hidden blessings and healing. The research evidence suggested that cultural safety was visible in practice in many diverse ways; it emphasised the complexity of the concept; accented its evolving status; and identified a relative consistency in defining cultural safety despite the varied contexts of practice.Although this study was limited by the small sample, the findings indicated that there were potential implications for nursing education, research, nurses and nursing practice as well as for other health care providers working in the current health care system. They suggested that actions from nurse educators, nurse managers, health care managers and clinical nurses themselves, would be needed to ensure that cultural safety continued to be part of nursing practice and contribute to the improvement of all health statistics in this country as well as to encourage an increased development in the focus on Maori health issues.Further nursing research suggested by the findings include studies to appraise cultural safety from a patient's perspective, and consideration given to the evaluation and assessment of nurses and their culturally safe practice. In addition, exploration and research could ascertain the benefits and rewards of culturally safe practice and identify ongoing educational needs as well as examining the views of other members of the multi-discipline team

  Abstract (up) This study investigated the degree of compliance and reasons for non-compliance with chemoprophylactic Rifampicin medication among 103 close contacts of memingococcal disease in the greater Auckland region during the period July to September 1996. The study showed a high degree of compliance among close contacts, with 96.1% taking Rifampicin as prescribed. Reasons for compliance were related to the contacts ' perceptions of susceptibility to the illness, the severity of the disease, and the benefits of medical intervention. The contacts found the service provided by the public health staff ws sufficient, and the information provided was valuable
